​If you are a DCYF social worker and are hoping to submit a referral, please complete the supplemental referral form linked on the right and submit it with additional referral documents to Dr. Swing's assistant Shayne via email: [email protected]. 

Private Evaluations

We currently have a 5-6 month waitlist for evaluations. If you would like to be added to our evaluation waitlist, please click on the appropriate link below to fill out the form to be added to our waitlist. 
Private Psychological, Neuropsychological, or Diagnostic Evaluations (e.g., for ADHD, Autism Spectrum Disorders, Learning Disorders, Diagnostic clarification, etc.) that you plan to have us bill your insurance for
Forensic or Court Ordered Evaluations (e.g., private parenting or custody related evaluations, court ordered evaluations, risk evaluations, etc.) that are not covered by insurance
